GUS Pizza Day: Pre-Pay

SEcond HALF OF THE SCHOOL YEAR

February 4, 2026 - June 3, 2026 (16 Wednesdays)

THIS FORM DUE: FRIDAY, January 30, 2026

Please write your student’s name and grade (5-8th grades, please include homeroom) and circle the number of slices per week and the corresponding amount.  ALL FORMS AND MONEY SHOULD BE RETURNED TO YOUR HOMEROOM TEACHER. CASH or CHECK ONLY (made out to: Glen Urquhart School, Memo: Pizza).

As a reminder, Wednesday is Pizza Day at GUS, where students can purchase slices of pizza for $2/slice. Pizza is ordered from Ellie’s pizza, and GF options are available. You may opt in to pre-pay or continue to bring cash.
